Audit Prep Checklist

Gather these documents before your audit to reduce stress and avoid surprises.

A) Payroll and Employee Records

  • Payroll summary by employee for the policy term (gross wages and overtime breakout)
  • Job titles plus a short description of job duties
  • Quarterly payroll tax filings used to verify payroll (e.g., federal Form 941)

B) Subcontractors and Contract Labor

  • Payments to subcontractors/contract labor (name, dates, amounts, work performed)
  • COIs for subcontractors (workers' comp, plus GL if required by contracts)
  • Material cost breakout where relevant (labor vs materials clearly separated)

C) Business and Financial Records

  • General ledger or accounting ledger for the policy term
  • Profit and loss statement for the policy term
  • Brief description of business operations and how work is performed

COI Quality Check (60 seconds)

  • Named insured matches subcontractor/legal entity name
  • Effective dates cover the full period of work
  • Workers' comp is listed (if required)
  • If additional insured is required, confirm the endorsement exists (not just COI wording)

A certificate of insurance is informational and does not change the policy. If you require insured status, you typically need the policy endorsement.

First 24 Hours After an Injury

Print this section and give it to supervisors. It reduces confusion and delays.

1Get care first. If emergency, call 911.
2Secure the scene to prevent additional injuries.
3Document the basics immediately: who, what, where, when.
4Gather witness names and contact info.
5Notify your internal point person (HR/Ops) the same day.
6Report the claim promptly per your carrier/administrator process.
7Start the return-to-work conversation as soon as appropriate.
